The username and password are located on the exterior packaging of your proficiency/competency test.

Forensic Assurance secure on-line answer sheet will allow you to record, save, review, print, and electronically submit your answers.

Below are some general instruction regarding how to navigate the online answer sheets.

Save Progress:

Your answers will be saved when you click on the “SAVE PROGRESS” or “NEXT” button at the bottom of each page.

Print / Review Your Answers:

The last page of the answer sheet allows the participant to review and print their answers prior to submission. The encryption code and title of any uploaded documents (STR Results Form or the Release of Results Form) will be displayed on this review screen, but the content of the uploaded document will not be able to be viewed.

Release of Results for Proficiency Tests:

The Release of Results form needs to be filled out, signed (this can be an electronic signature) and returned to Forensic Assurance if the results are to be released to your accreditation body.  The participant can upload the Release of Results form on the last page of the online answer sheet, or the form can be emailed to Forensic Assurance at results@forensicassurance.com.

Confirmation Email:

The participant can enter an email address on the final page of the answer sheet. Once the answers are submitted, an email confirmation (HTML format) will be generated and sent to the email address that was provided.  This confirmation is auto-generated and is sent immediately after the answers are submitted. There will be no further correspondence provided to the email address that was provided on the first page of the answer sheet.

Reports for Proficiency Tests:

You will receive two reports from Forensic Assurance.

The Report of Proficiency Testing Results will describe the design of the test, the correct answers, and the participant’s results.  This report is provided approximately 30 days after the Answer Submission Deadline, which is posted on the Forensic Assurance website as well as the first page of the online answer sheet.

All Reports of Proficiency Testing Results that are issued by Forensic Assurance are provided to the individual at your laboratory that placed the order (this is typically the Quality Manager, a Technical Lead or Laboratory Director).  The results will not be released to any other individual without written consent.

The Statistics Report will provide the cumulative results of all participants who took the test.  Information included in this report will include the false positive error rate, false negative error rate, sensitivity and specificity.  This report will be provided approximately 60 days after the Answer Submission Deadline.

These reports will not be released to the email that was provided on the first page of the online answer sheet without written consent from the purchaser of the test.

Reports for Competency Tests:

You will receive one report from Forensic Assurance.

The Report of Competency Testing Results will describe the design of the test, the correct answers, and the participant’s results.  This report is provided approximately 4 weeks after the answer are submitted.  These reports will be provided to the individual at your laboratory that placed the order (this is typically the Quality Manager, a Technical Lead or Laboratory Director).  The results will not be released to any other individual without written consent.
